CHALLENGES IN PUBLIC HEALTH- A BRIEF INSIGHT INTO ADVERSE REPRODUCTIVE OUTCOMES
Short Abstract
:
Maternal child health and reproductive health is one of the major thrust areas of the national health policy. Though access to reproductive health services has improved over the decades and accessibility also has spanned over the country quite well, but negative reproductive outcomes like still birth, miscarriage, congenital birth defects do not get the focus and attention it requires, specially considering the physical and mental health of the mother with adverse reproductive outcome. Influence of socio economic factors contribute much to a successful reproductive outcome. Focus should be given on the mental and physical health of mothers who have suffered adverse reproductive outcome as a public health policy.
Long Abstract
:

For a positive Birth outcome, influence of bio genetic factors is very crucial. Emotional stress in a pregnant woman operationally defined by the factor like unwanted pregnancy might interfere with fetal development and result in higher increase of malformations. The interferences may occur directly via psycho endocrinal as autonomous mechanism or indirectly by the way in which a woman might act in a situation of stress, i.e. attempts to provoke abortion, neglect of proper food intake, excessive smoking or abuse of alcohol and drugs. Experiencing one stressful event like job loss, separation, divorce or loss of any one close to them during periconceptional period was associated with varied adverse reproductive outcomes even to women who were not obese and women with less than equal to high school education. Women who experienced stressful life events around the time of conception or early gestation might be at increased risk of delivering infants with certain congenital anomalies.Malformations recognised at birth are predominantly of types which are suspected to have complex genotype as well with environmental contributions to their etiology, and the nature neither of the environmental nor of the genotypic contributions to their etiology is well understood. There is ample evidence all over the world that over all frequency of malformations present at birth would be increased by about 50% if proper documentation can be done. Epidemiologic studies have thrown light on associations between maternal exposure to air pollution in the form of pre term births, pregnancy complications like preeclampsia and gestational diabetes. Different observational studies have been carried out which has stressed on the importance of bio-genetic and socioeconomic factors influencing reproductive outcome and ultimately having impact on public health of the future global citizens. Public health policies must stress on proper documentation of reproductive outcomes to address the issues.
